Minor Requirements
All of the documents are required at every minor piercing or tattoo appointment, regardless of age.
Anyone under 16 can Not be Tattooed, and for piercings MUST be accompanied by their parent or legal guardian during the procedure.Minors 16 or older still need all legally required documentation and notarized consent, but guardian does not need to be present.Florida law dictates only the minor's natural parent or legal guardian may provide consent. Under Florida law, anyone under 18 wishing to be pierced (yes, even lobes!) must provide:Photo identification for consenting guardian AND minor (like valid, non-expired state ID, passport, or public school-issued ID or yearbook)
Proof of age for consenting guardian AND minor (like valid, non-expired state ID, passport, or birth certificate)
Proof of legal relationship to the guardian signing consent (like original or certified copies of birth certificate, custody paperwork, adoption paperwork, or guardian paperwork)
